All about revision surgery

Breast implant technology and surgery is constantly evolving and improving, however it is likely that all breast implants will need to be revised during the course of a patients life, particularly young patients. Breast revision surgery can mean removing the breast implant without replacement or removing and replacing the implant.

Reasons why a patient may consider breast implant revision surgery:

During your consultation, Dr Wessels and our breast Nurse will evaluate your breasts and body type to develop the best treatment plan for you. Utilizing our revolutionary VECTRA® 3D imaging and assessment technology, you will be able to visualize the outcome of your breast procedure and determine which augmentation approach can meet your expectations. Breast Implants

At One Cosmetic we use the highest quality breast implants available on the Australian market being Motiva and Mentor. Breast Implants come in either a textured or smooth surface shell and are available in a wide range of sizes and profiles to fit the patient’s body. Each silicone implant is filled with a cohesive gel that holds together uniformly and retains a natural give that better resembles breast tissue. All procedures aims to strike a unique balance between selecting an implant most appropriate to body type while addressing patient expectations. At One, we use Mentor and Motiva breast implants due to their high safety rating and patient satisfaction. These brands have a high safety rating due to their nano-textured (Motiva) or micro-textured (Mentor) surface.

Mentor breast implants come in either a round or teardrop shape. Round breast implants are available in low and high profile whilst the tear-drop shaped breast implants are available in various widths, heights, projections and sizes. Generally, round implants are more suited to patients that are happy with the breast shape and are looking only to increase in the size of their breasts. Round implants give more upper-pole fullness to the woman’s breast.

On the other hand, tear-drop implants give a much more natural shape to the breast. The more projected lower half of the breast lifts the nipple and may suit those patients with slightly droopy breasts, such as post-pregnancy, breastfeeding or with weight loss.

Motiva breast implants are available in a round or ergonomic shape. Round Motiva breast implants are devices composed of a shell, a patch, and a cohesive silicone gel fill. They have balanced gel elasticity and firmness, upper pole fullness, and provide a younger active look

With the ergonomic breast implant, patients get the look, feel and movement of a natural breast. It maintains its softness over time and has proven its efficacy after years in the market. The ergonomic implants is designed to mimic a natural breast, when laying down the implant will give a fuller round appearance, when standing the lower pole of the breast will be fuller as this implant has more movement.

The Surgery & Recovery

Before your procedure, Dr Wessels and our surgery team will make sure that you are comfortable and well before going ahead. Dr Wessels will draw on your breast to plan the ideal placement of the breast implant, as well as the intended incision used to place the breast implant.

The breast augmentation is performed under general anaesthesia in a fully accredited private hospital with the assistance of a qualified anaesthetist. Breast augmentation takes 90 minutes in total, a small incision is placed under the fold of the breast and the implant is then carefully inserted under the breast tissue or under the pectoral muscles.

Once the implant is correctly in place, the incision is carefully closed in layers with a concealed absorbable stitch. The incision is then covered with a waterproof dressing. Patients are then taken to recovery for 2-4 hours with our recovery care specialists. You will be allowed to travel home that evening, as a breast augmentation is a day surgery only.

Recovery

After surgery our recovery specialists will given post-op instructions. It is important to follow carefully. You will be provided with a script for medication, which you will need to purchase after surgery and instructions on when to take the medication over the days ahead. You will be discharged from hospital when your companion picks you up from the hospital. All patients must have a companion to stay with them for 72hours following surgery.

The first week after surgery you should stick to gentle day-to-day activities. This includes minimal house duties, no lifting, staying home as much as possible and sleeping in an elevated position. Commence postoperative exercise (instructions given post surgery).

In the second week of recovery try not to raise your blood pressure too much and still sleep in an upright position. Patients will also be required to attend a postoperative appointment at our office in week 2. this will be scheduled prior to surgery.

 

In week 2 patients may:

  • Commence a light walk

  • Return to work in week 2 if you have an office job with little physical activity

  • Avoid sexual intercourse

  • Start driving provided they are not taking any pain medication. We also recommend waiting 3 days for the medication to be out of your system.

Weeks 3 – 5 patients with more physical jobs can return to work.

 

In weeks 3-5 patients may:

  • Start getting into a light exercise routine, using lower body

  • Start doing more physical household duties

In weeks 6-8 patients may:

  • Commence normal exercise routine and duties. Please note, do not push yourself into being back into your pre surgery routines.

  • Swimming, bathing running, yoga, stretching, spin class and Pilates

  • Stop wearing compression bra (Textured Mentor implants)

At this stage patients are mostly fully recovered and sutures are absorbed. Patients should always wear a supportive bra when exercising or performing physical activity. For all patients with teardrop breast implants we recommend that you do not ever go back to using or exercising chest muscles heavily as teardrop implants can rotate.

In weeks 9-12 patients may:

  • Contact sport

  • Boxing and upper body exercise

  • Stop wearing compression bra (smooth Motiva implants)

Capsular contraction is a capsule of scar tissue. This can become thickened and result in breast firmness. The exact cause is unknown but factors that may lead to this complication include seroma (development of extra fluid around the implant), haematoma, infection and smoking. Most cases will need further surgery.

The breast implant can interfere during a mammogram as it can hide suspicious-looking breast tissue, making it difficult to interpret results. At ONE we have a breast implant check clinic that is free to our patient’s post-procedure. Our clinic specialises in breast implant checks post procedure.

Sensation in the nipple can increase or decrease post procedure, it can also be temporary or permanent.

The implant can shift from its initial placement resulting in further surgery required. Breast implants can also rupture, this may result from natural ageing of the implant or excessive compression or trauma. Saline can be absorbed by the body. Silicon implant leaks are either intracapsular or extracapsular (into the breast tissue). As the gel is now the consistency of thick sticky toothpaste, it often remains in the implant even with a rupture.

  • Surgical risks such as bleeding or infection

  • Fluid accumulation around the implant after surgery

  • Allergic reaction to suture materials, tape adhesive or other medical materials and lotions

  • Changes in breast and nipple sensation

  • Temporary or permanent areas of numbness

  • Wrinkling of the skin over the implant

  • Keloid, or lumpy scar tissue, which is pink, raised and irregularly shaped. These scars may be inflamed and itchy. There are several possible sites for the incision. Discuss this with your surgeon

  • Inappropriate implant size

  • Asymmetry (unevenness) of the breasts

  • Calcium deposits in the scar capsule around the implant

  • Granulomas, or lumps in local lymph node tissue formed by leaking silicone

  • Breastfeeding difficulties, including reduced milk supply

  • Reduced effectiveness of breast cancer screening, since an implant may hide breast tissue (and tumours) during a mammogram

  • Movement of the implants from their original position

  • Further surgery to treat complications

  • Risks of anaesthesia including allergic reaction or potentially fatal cardiovascular complications such as heart attack

  • A blood clot in the deep veins of the legs (deep vein thrombosis), which can move to the lungs (pulmonary embolus) or to the brain and may be life threatening

Monitoring your safety for Life

Lifelong Breast Implant Surveillance Program

With any surgery, follow-up care is essential. So, we provide patients with ongoing support through our Lifelong follow up. All patient who have breast surgery with us will receive regular post-operative check-ups to avoid any potential complications in the medium to long term. It is important to understand that a medical device has been used and must be monitored throughout the time the patient has the implant.

Patients who decide to undergo a breast augmentation must understand that in the future they will need to have further surgery as breast implant do not last forever. Future surgery is often required to replace one or both implants. As with all surgical procedures, revisional surgery may also be necessary to correct any problems that may develop.
The Breast Implant Registry (BIR) is a public health initiative focusing on patient safety. The Registry collects relevant patient, surgeon, procedural and implant data relating to breast implants. We register all patients post surgery. The Registry is administered by the Australian Society of Plastic Surgeons Inc (ASPS).
Motiva
All Motiva implants are covered against rupture by the Always Confident Warranty, for the lifetime of the device, and by Product Replacement Policy against capsular contracture, Baker grades III and IV for a period of 10 years.   Furthermore, Motiva offer Extended Warranty Programs, which you may decide to participate according to your personal case:  
  • For Motiva Implants®without Q Inside Safety Technology™ our 2Y Motiva Program™
  • For Motiva Implants®with Q Inside Safety Technology™ our 5Y Motiva Program™  
In addition to the replacement product, these programs will support you with financial assistance for each affected implant, applicable to the cost of the revision surgery in the event of a warranty claim for rupture or capsular contracture Baker grades III and IV.
Mentor
All recipients of MENTOR® Breast Implants are automatically enrolled in the MENTOR® Promise Protection Plan. Should a confirmed rupture or deflation occur, the patient will receive up to two replacement breast implants of the same style or like style at no additional charge for the lifetime of the patient. If a rupture occurs within 10 years of surgery, the patient may also be eligible for financial assistance for surgical costs not covered by insurance. Additional complications are also covered for 10 years with free product replacement.   Implantation of the MENTOR® Breast Implants, as well as any subsequent procedures, must be in accordance with current Mentor Product literature and accepted plastic surgical procedures by appropriately qualified licensed physicians for product to qualify for replacement under the Mentor Product Replacement Policy.
Frequently Asked Questions

We regularly get asked about our procedures, so we have put together a couple of questions we get asked on a regular basis. If there is something else you would like to know, please get in touch!

Individual cases may vary but generally, mothers can breastfeed after having breast augmentation. However, some women do experience reduced nipple sensation following breast augmentation surgery making it difficult to trigger the milk let down reflex. Some women may also experience reduced milk supply.

Unfortunately, breast implants will not last forever. Life expectancy is estimated at 10-15 years.

As this is a cosmetic procedure, there is no Medicare rebate or health insurance cover available.

Scars are apart of this procedure. The scar is about 3-4cm under each breast fold. With care and diligence the scar will fade over time.

Dr Adam Honeybrook

US DOUBLE BOARD CERTIFIED IN FACIAL PLASTIC AND RECONSTRUCTIVE SURGERY

Dr Honeybrook completed a 6-year medical degree at the University of New South Wales (UNSW), Sydney, in 2009. He then completed basic surgical training at Royal Prince Alfred Hospital, Sydney, whilst also sitting the United States Medical Licensing Examinations. This provided Dr. Honeybrook the unique opportunity to train at top hospitals throughout the United States.

 

From 2012 onwards, Dr Honeybrook undertook an impressive amount of training and accreditation in the United States, including General Surgical training at Vanderbilt University Medical Centre, a highly selective 5-year specialist training position in Head & Neck and Facial Plastic Surgery at Duke University Hospital in North Carolina – which encompassed extensive subspeciality training in Facial Plastic and Reconstructive Surgery, including microvascular tissue transfer, oncologic facial reconstruction, maxillofacial trauma reconstruction, facial reanimation and reinnervation procedures and facial aesthetic surgery – followed by an additional 2 years of accredited fellowship training dedicated purely to Facial Plastic Surgery via the prestigious University of Pennsylvania Facial Plastic and Reconstructive Surgery fellowship under the auspices of the American Academy of Facial Plastic and Reconstructive Surgery (AAFPRS).

 

In 2019, Dr Honeybrook completed further fellowship training in Facial Plastic and Reconstructive Surgery in Louisiana and became an Assistant Professor of Head & Neck and Facial Plastic Surgery at Louisiana State University.

 

Dr Honeybrook then returned to Australia in 2020 to work as a specialist consultant surgeon at Royal Brisbane and Women’s Hospital and he was inducted as a fellow of the American College of Surgeons in 2021. In 2022, Dr Honeybrook was inducted as a Fellow of the Royal Australasian College of Surgeons, and he was also granted Board Certification in Facial Plastic and Reconstructive Surgery by the American Board of Facial Plastic and Reconstructive Surgery.

Dr. Sepehr Lajevardi

Dr Sepehr Lajevardi is a specialist plastic and reconstructive surgeon specialising in aesthetic surgery of the breast and body. After undertaking his undergraduate medical training at University of NSW with a scholarship, he went on to undertake 12 years of post graduate 12 years of post graduate training to be recognised as a specialist plastic surgeon with the Royal Australasian College of Surgeons. Dr Lajevardi has a keen interest in research with many publications and more than 50 presentations at national conferences. Dr Lajevardi was also mentored by many of Sydneys previous leading plastic surgeons to further his skills in aesthetic plastic surgery.

 

Dr Lajevardi is a clinical associate lecturer at the university of Sydney. Dr Wessels received his MBChB in South Africa, completing his internship and general surgery residency, training in one of the most challenging clinical environments, before completing his Fellowship and his Master of Medicine in Plastic and Reconstructive Surgery.

Dr Wessels successfully completed his second Fellowship in Plastic and Reconstructive Surgery when he was awarded a Fellowship of the Royal Australasian College of Surgeons in 2015. He has published world leading research on breast implant infection and outcomes following breast augmentation surgery.
