Dermal Fillers for the Tear Trough Area
The tear trough refers to the depression located between the lower eyelid and the upper cheek. In some individuals, a loss of soft tissue volume in this area can become more pronounced over time due to the natural aging process or genetic factors. This can result in a concavity that creates a shadow under the eyes.
A dermal filler procedure for this area involves the injection of a gel-like substance, typically containing hyaluronic acid. This product is placed in specific points within the tear trough to add volume. By adding volume to the depression, the treatment can reduce the shadowing caused by the concavity. The dermal filler product itself may also have a hydrating effect on the skin in the treatment area.
The tear trough is a complex anatomical region, and treatment is not suitable for everyone. A thorough consultation with a qualified medical practitioner is essential to assess your anatomy, determine if this procedure is appropriate for you, and discuss the potential risks and complications involved.
In most tear trough treatments we complete the treatment in stages. It is very important to not to rush the treatment for a quicker end result. The skin around the eye is thin and delicate, therefore placing filler in this area can be tricky for some injectors. When injecting the tear trough and under eye area with dermal filler, it is necessary to inject multiple areas to obtain the best results, and we also prefer to inject over 2 sessions to avoid the very common negative results of tear trough treatment, which include lumps and blue appearance (Tyndall Effect).

Both treatments involve thoroughly cleaning the area first. A topical anesthetic will be applied for your comfort. The treating doctor or nurse will then use a cannula to administer the filler. At One Cosmetic, we use a cannula in this area as it is a safer administration method. The eye area is considered a high risk area, however the use of a cannula decreases the risk dramatically. The first treatment will involve a small amount of filler placed in the area, you may not have an immediate noticeable difference after the first treatment, which is a good thing. This means your injector has laid the structure for the tear trough. The second treatment will be the same process, however usually less filler is injected.
Your tear trough appointment will take about 45 minutes from start to finish. As soon as the series of injections is completed, you can go back to your regular activities.Â

These products also contain lidocaine, which means that the treated areas are numbed simultaneously during treatment, improving the overall treatment experience for the patient. Your injector will then gently massage the fillers to soften the product.

Dr. Louis Wessels is a highly qualified specialist plastic, cosmetic, and reconstructive surgeon with a particular focus on breast and body surgery, including liposuction.

His practice encompasses the full spectrum of cosmetic and reconstructive procedures, with special expertise in breast surgery. These include primary and revision breast augmentation, breast reduction, breast lift, and tuberous breast reconstruction, as well as body contouring procedures such as abdominoplasty (tummy tuck) and liposuction.

Dr. Wessels completed his medical degree (MBChB) in South Africa, followed by internship and general surgery training in one of the most demanding clinical environments. He subsequently completed both his Fellowship and Master of Medicine in Plastic and Reconstructive Surgery in South Africa, gaining extensive experience across complex reconstructive and cosmetic procedures.

He later completed a second Fellowship in Plastic and Reconstructive Surgery in Sydney, Australia, and was awarded Fellowship of the Royal Australasian College of Surgeons (FRACS) in 2015. Dr. Wessels has published internationally recognised research on breast implant infection and outcomes following breast augmentation surgery.

He subsequently graduated from Macquarie University in the inaugural intake of the Master of Advanced Surgery (Plastic and Reconstructive Surgery), for which he was awarded a scholarship from Johnson & Johnson.
